Securing the muscle using 6-0 vicryl

This video demonstrates our technique for securing the muscle prior to resection, using 6-0 vicryl sutures. Here the muscle is stabilized using 2 muscle hooks and slightly elevated from the sclera. We are using a double-armed suture. One of the needles is woven through the full width of the muscle and the suture pulled through until the two ends are of equal length. Locking bites are the placed at each end of the muscle. We accomplish this by taking a partial width bite through the muscle near the superior border and pulling the suture through until we are left with a small suture loop above the muscle. We grasp the loop with forceps and pass the needle back through the loop twice and pull the suture taught while ensuring correct positioning from the insertion with the calliper. The same process is repeated for placement of the locking bite at the inferior border.
