Immunization Record:  (To open form in PDF format click here):  Your base hospital has asked McGill University to assist in collecting this important document.  Responsibility for interpreting the medical data resides with the hospital.  Returning trainees who have already completed the process only need to fill out the "TB" section and then to submit the form to their base hospital.  This is required of all McGill Residents and Fellows yearly in order to be in good standing and registered, but also in order to commence work and be paid.  To find out where you are based, please go to the Resident ONLY site to view your Virtual Training Card.
