The following information must be submitted to the office of Professional Services for Rouyn-Noranda (attention Denise Paquet 819-976-2900), prior to arriving in Rouyn:
- Proof of Membership from the Canadian Medical Protective Association 1-800-267-6522
- Proof of Membership from the College des medecins 1 888 MÉDECIN
- Copy of your CV + 2 letters of reference
- Completed form “Démande de nomination de médecin, dentiste our pharmacien en CH”
- Completed form RAMQ 2880 for Hematology privileges
