Procedures to follow :
The requesting physician must fill out the following form Request for Appointment and fax it along with the following documents to Carole Turcotte at 514-340-8129:
Pathology;
X-ray reports;
OR reports (if available);
Blood if abnormal.
Please contact Carole Turcotte at 514-340-8248 prior to faxing your request so that she can keep an eye out for it.
Once Carole Turcotte has received the correct information she will schedule an appointment for the patient and contact the requesting physician with the time/location.
It is the responsibility of the requesting physician to contact the patient and provide them with the appointment information. Please refer to our Patient Information Sheet for the exact coordinates of each hospital and appointment location.
