SGAS Forms

These forms are supposed to be used to request cardiac interventions (cath, pacemakers, ICD, EPS, cardiac surgery). They are a project of the MSSS and the RQCT. This includes hospitalised patients also. The problem is a lack of computer database access and personnel at present, but get used to using them.
